Referrals
It is your responsibility to arrange for a referral form before your visit. Without a referral, you will not be seen. You may fax your referral form from your primary physician’s office to our office. This must be done before your appointment.

Payment Information
All co-payments and deductibles are due at the time of the visit. If you have an indemnity plan or your policy allows you to go “out of plan”, you can be treated here. For nonparticipating and secondary insurances we will provide the necessary receipts, forms and codes to make claim filing easier. Payment for services is required the day services are provided.

Please bring your insurance card and a photo ID on the day of your appointment.

The patient (or legal guardian) is responsible for any deductible, co-payment, or portion of a bill that is not otherwise covered by insurance.
